Induced Ischemic Arrest of the Heart: Metabolic Considerations

چکیده
Normothermic ischemic arrest of the heart as an adjunct to open-heart surgery has gained widespread clinical acceptance. This form of arrest, achieved by simply cross-clamping the ascending aorta to eliminate coronary perfusion, is convenient in that it provides a bloodless operative field and a relaxed myocardium. The upper limit ofthe safe period of such ischemia is generally considered to be in the range of 30 to 45 minutes. 1 Clinical experience has revealed that, in some cases, it is possible to exceed these limits with relative impunity. However, about half of the deaths during cardiac surgery and in the early post-operative period are associated with some degree of ischemic necrosis of the inner third of the myocardium, the characteristic lesion of prolonged ischemic arrest of the total heart. 2 , 3 • Safety in elective arrest of the heart thus becomes a problem of great importance in cardiac surgery.

Recovery from cardiac bypass and elective cardiac arrest. The metabolic consequences of various cardioplegic procedures in the isolated rat heart.
Isolated perfused working rat hearts were subjected to elective cardiac arrest for 20 or 30 minutes. Various methods of arrest, either singly or in combination and with or without coronary perfusion, were studied. The functional recovery of the heart following the termination of arrest was related to the concentration of adenosine triphosphate (ATP) and creatine phosphate in the myocardium at t...
متن کاملA New Method for On-Farm Euthanasia with Animal Welfare Considerations
Objective: Euthanasia of animals is a highly stressful situation for veterinarians and may be source of ethical dilemmas; also, it is very difficult to decide “which technique is more appropriate for the given situation”? The aim of this study was to describe a new humane method for on-farm euthanasia. Design: Pilot clinical trial. Animals: thirteen animals including three horses, six sheep ...
متن کاملEffects of Parenteral Vitamin D on the Biomarkers of the Endothelial Function in Patients with Type 2 Diabetes and Ischemic Heart Disease: A Randomized Clinical Trial
Vitamin D deficiency is associated with cardiovascular and metabolic diseases. Cardiovascular diseases, in turn, are responsible for mortality of patients with type 2 diabetes (T2D). We investigated whether a single parenteral dose of 25(OH) Vit D could improve the endothelial function in T2D patients with ischemic heart disease. A randomized, placebo-controlled, and double-blind trial was perf...

Hypothermic potassium cardioplegia is now commonly used to protect the myocardium during surgically induced ischemia. Because the potassium-related membrane depolarization has been shown to increase calcium influx, we undertook this study to define the effects of varying the calcium content in hyperkalemic perfusates and the effects of using magnesium instead of or in addition to potassium as t...
